Diagnostic dilemma: Rash 'migrated' across man's body from his anus

Patient: 64-year-old man from Spain.

Symptoms: A patient previously diagnosed with lung cancer needed to be hospitalized because the malignancy had spread and was pressing on his spinal cord. He was given high doses of glucocorticoids, which are used to reduce inflammation and are sometimes used as part of cancer therapy or to ease the side effects of chemotherapy. However, four days later, he developed an itchy rash and mild diarrhea.

What happened next: Doctors examined the rash, which looked like red, wavy lines on the patient’s body. The clinicians noted that the rash first appeared in the anal area and then “rapidly spread to the trunk and extremities,” they wrote in their case report. During the initial examination, the doctors outlined the rash with a pen, but within 24 hours, the rash had “moved from its original locations.” In other words, it was no longer within the pen outline.
